Attached are some patches to fix a failover issue that people noticed.

In a cascaded configuration with multiple nodes failing at once it was 
possible to have a self-subscription in sl_subscribe.

This is fixed, also we were not reloading set origin information after a 
FAILOVER event in slon.

I think this fixes the issues.
