TABLE ADD KEY is no longer supported in 2.x and yet slonik creates scripts
using it. This results in an obscure bug that is hard to nail down until one
realizes what the problem is. TABLE ADD KEY should probably also be expunged
from the 2.x docs when it is removed from slonik generated scripts.
